On your first visit, you will be asked to fill out a new patient form which will tell us more about your health history. Then you will meet the health care provider to discuss your current health situation and to see if you’re a good candidate for our care. If we think we can help, we’ll conduct a thorough examination. This helps us identify the likely cause(s) of your problem. We’ll tell you what we found, what we can do to help, how long it may take and how much it will cost. Please bring the following:
  • If you have had Blood work, X-ray/MRI/CT or any other examinations recently and have access to the reports, please bring a copy.
  • A list of current medications and dietary supplements (include vitamins and minerals) you are taking, including the dosage or bring the actual prescription bottles.
  • A list of other physicians or other locations of care you may currently be using.
